Work Quarantine Pass (Please Read)

  • This form is for the replacement of existing Work Quarantine Passes.
  • Please fill in the necessary information to request for the new Centralized  Work Quarantine Pass. 
  • You will be notified via text message once your pass is ready.
  • Once you get a text message, please book an appointment for pick-up on the Pilar Shield Facebook page by clicking "BOOK NOW" and wait for confirmation of appointment. (This is to control the number of claimants per date and prevent mass gathering).
  • You must bring your Company ID or COE and your old Work Quarantine Pass to surrender in replacement for the new pass.
  • Applicants must get their requested pass personally as picture taking is required.  If unavailable, you can ask someone to get it provided that you issue an Authorization letter and your ID with Picture (Physical ID).
  • Claiming without the above mentioned requirements will NOT be released, whatever reason it may be.

NOTE: 

                   All data collected will be used ONLY for contact tracing purposes and will be shared ONLY to the relevant department.

          ALL OLD QUARANTINE PASSES WILL BE INVALID BY THE END OF OCTOBER.
