Student Quarantine Pass (Please read)

• Please fill in the necessary information to request for a Student Quarantine Pass. 
  
• For University Students, please bring your School ID or School Admission slip to claim your Student Quarantine Pass.
  
• We will not release the Quarantine Pass if no proof of study is presented.
  
• Pilar Elementary and High School Students are already verified through school records we acquired.
  
• We will not process applications if names are not on the School records.
  
• You will be notified via Text or Email once the quarantine pass is ready. 
  
• Once you get a text message, please book an appointment for pick-up on the Pilar Shield Facebook page by clicking "BOOK NOW" and wait for confirmation of appointment. (This is to control the number of claimants per date and prevent mass gathering).
